Young people with complex health needs have a particular hard time when it comes to making the transition from children's to adult services. The difference between health systems for children and adults makes the move from one to another particularly hard for young people with complex health needs. In addition to this, many health trusts and PCTs remain reluctant to release funding for use as a Personal Budget. Both these factors make it particularly difficult for disabled young people with complex health needs to shape their lives in ways they would like.
As part of our work developing Personalised Transition, ibk is currently consulting with families about their experience of a) health services at transition and b) access to personal budgets through health.
