A parent's perspective on health at transition

The following powerpoint presentation was put together by a parent - Marta Cohen - one year after her son left school. Unfortunately, in spite of extensive knowledge of the health services, Marta has been unable to arrange for all the things her son needs to be put in place. The presentation outlines the difficulties many young people with complex health needs face at the time of their move from childrens to adult health services.
